Description

One person dies every 2 minutes daily in the U.S. from sepsis. Survivors can experience cognitive, mental, and functional deficits long after the acute illness has resolved. Many organizations have initiated programs around the septic patient, but these programs fall short of desired outcomes. As of October 2015, the Centers for Medicare & Medicaid Services (CMS) has made sepsis a core measure. The complexity of the disease, the variability in the presentation of the patient, the skill and knowledge level of the provider make this core measure distinctly different and potentially a greater challenge to implement. To provide the right care to the right patient at the right time across the continuum, systems and processes need to be well defined. This webinar workshop will focus on taking the learner through a four-tiered strategy for ensuring a robust program to identify patients early in the disease and manage them based on the latest evidence. The presenter will reveal key implementation strategies for hard wiring the clinical and process behaviors. In addition, the presenter will describe barriers with reliable actions to reduce their impact, and participants will have the opportunity to create a test of change action plans to help address those barriers.

Objectives:-

Part 1: Sepsis Update 2020: Incidence, Mortality and Bundle Science Update

Thursday, June 24, 11:00 AM EST - 01:00 PM EST, 120 mins

  • Determine the impact sepsis has on mortality, location of disposition, and long-range economic impact
  • Examine any new evidence on the sepsis bundles and implementation.

Break: 01:00 PM EST - 01:30 PM EST

Part 2: Getting the Team to Play: Strategies for Increasing Team Member Buy-In

Thursday, June 24, 01:30 PM EST - 03:30 PM EST, 120 mins

  • Examine methods to increase physician participation to ensure a comprehensive approach to recognition and management of the septic patient
  • Determine strategies to obtain leadership buy-in for the need and actualization of a sepsis coordinator role
  • Identify and work on an intervention for physician buy-in that participants can start by next week to implement, using the test of change framework.

Break: 03:30 PM EST - 04:00 PM EST

Part 3: Finding the Septic Patient: Challenges and Solutions to Early Identification

Thursday, June 24, 04:00 PM EST - 06:00 PM EST, 120 mins

  • Identify locations and processes for screening by the front line
  • Evaluate secondary recognition strategies and the role of the rapid response in early recognition.

Part 4: Addressing the Challenges

Friday, June 25, 01:00 PM EST - 03:00 PM EST, 120 mins

  • Examine the evidence and practical application for fluid challenges
  • Identify strategies to improve the timeliness of antibiotics, blood cultures, and lactate.

Break: 03:00 PM EST - 03:30 PM EST

Part 5: Septic Shock: Vasopressors and Precision Driven Fluid Resuscitation???????

Friday, June 25, 03:30 PM EST - 05:30 PM EST, 120 mins

  • Discuss the rationale for use of central lines in patients with septic shock
  • Compare and contrast the methods to assess volume status (CVP/SCVO2, focus assessment, Dynamic measures of perfusion).
